You must complete a form for each course you are enrolled in. Click the SUBMIT button and then click RETURN TO THE FORM if you are enrolled in more that one online course. You should print a copy of the "Form Confirmation" notice for your records.

your first initial, your last name, and the last four numbers of your SSN. For example, if your name is Jane Doe and your Social Security Number is 123-45-6789, then your email log in is: jdoe6789. Your password is the first six numbers of your SSN: 123456.
